Minor Release Form for 2016
Please note: All minors in the pits whether they are a participant or a spectator must have a minor release form filled out along with a copy of their birth certificate on file with US 30 Speedway.
Participants must have their form signed by both parents and notorized as such. US 30 Speedway will have a Notary on site for a fee of $5.
Form is available on www.usthirtyspeedway.com or at the Speedway.
